The hours of this position are Monday through Friday 3:
00 PM through 9:
00 PM and On-Call responsibilities (emergent visits only, not triaging calls) from 9:
00 PM until 8:
00 AM the following morning, Monday through Thursday.
An RN Assessment Nurse will:
Administer skilled nursing care to patients requiring professional nursing service.
Participate in patient and family education and communicate with other members of the Care Delivery Team.
Exhibit competency with the OASIS Assessment document.
Function as a Care Delivery Team member, focusing on the shared agency and Care Delivery Team goals of an accurate, complete OASIS Assessment, appropriate Case Mix, proper utilization of services, & patient-centered care to ensure accurate & appropriate levels of care.
Conduct initial assessment and admission of patients to the agency using a patient-centered approach.
Demonstrate the coordination of the Start of Care admission process through timely, accurate communication and the establishment of trust and respect with the receiving care provider team.
Adhere to the scope of practice for Registered Professional Nurse per New York State regulatory guidelines.
Assume responsibility requirements and have a comprehensive understanding of various insurances.
Accurately complete and submit ALL required documentation within established agency standards and guidelines.
Provide and coordinate quality patient-centered care, maintaining productivity.
Obtain a medical history, particularly as it related to the patient's present condition from the patient and/or designated caregiver.
Conduct a comprehensive assessment of the patient including vital signs, physical and mental status.
Demonstrate critical thinking skills to be able to make clinical decisions for delivery of safe, effective, efficient, high quality patient care in collaboration w/the MD.
Observe and assess signs and symptoms of any adverse findings and immediately reporting to the physician including, but not limited to, reactions to ordered treatments, medication interactions, and any changes to the patients physical and/or emotional status.
Demonstrate competent nursing skills, treatments, preventative procedures, knowledge, and judgment necessary to implement physician orders, nursing procedures, and nursing interventions.
Develop and implement a safe, realistic, individualized Plan of Care for each patient in collaboration with other services/disciplines according to best practice standards to promote positive patient outcomes and patient satisfaction.

Working conditions may expose field staff to disagreeable sights, odors, body wastes, communicable diseases and blood borne pathogens, smoke, pets, and other allergens.
Potentially hazardous materials such as those found in cleaning agents and other chemicals may be stored in a client's home.
Required Skills Strong computer skills and the ability to use Agency's EMR program.
Strong interpersonal, organizational, time management and written and verbal communication skills.
Able to work independently, and complete paperwork accurately and in a timely fashion.
Must comply w/privacy & HIPPA laws.
Must be respectful of the patient's rights & treat a client & his/her belongings with dignity & respect.
Valid driver's license and automobile with current insurance coverage and in good working condition available to use for work.
Must be willing to travel throughout assigned county.
Willingness to submit to physical examination, criminal background and drug testing.
Required Experience Registered Nurse with a current NY State License.
Bachelor's degree from an accredited college preferred.
Minimum of 1 year Home Health Care experience, specifically with OASIS-C, strongly preferred.
Job Location Plattsburgh, New York, United States Position Type Full-Time/Regular.
Estimated Salary: $20 to $28 per hour based on qualifications.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
