Occupational Therapist - Plattsburgh, NY
The primary purpose of this position is to provide occupational therapy screens, initial evaluations and treatment per therapy plan of care and to communicate this information to the interdisciplinary team.
Completes Occupational Therapy evaluations upon physician order.
Establishes Occupational Therapy plan of Care for treatments per physician order.
Modifies treatment program level, intensity and techniques based on the resident's response to specific treatment plans.
Completes screens/ evaluations of facility residents, makes appropriate recommendations for nursing regarding mobility, positioning and ROM.
Completes resident progress notes per facility policy.
Comprehensive pay & benefits package
Must possess a current unencumbered NYS license as a Registered Professional Occupational Therapist, Bachelor's degree or higher.
Experience with the MDS, RUG's, Medicare guidelines and JCAHO criteria.
Geriatric experience preferred.
A current unrestricted driver's license and dependable transportation.
