Rehabilitation Manager - Plattsburgh, NY
The primary purpose of this position is twofold:
to provide supervision and direction to the rehabilitation staff and participate as a member of the facility management team and to complete therapy screens, initial evaluations and treatment per therapy plan of care and to communicate this information to the interdisciplinary team.
Completes Therapy evaluations upon physician order.
Establishes Therapy Plan of Care for treatments per physician order.
Modifies treatment program level, intensity and techniques based on the resident's response to specific treatment plans.
Completes screens/ evaluations of facility residents, makes appropriate recommendations for nursing regarding mobility, positioning and ROM.
Completes resident progress notes per facility policy.

Must possess a current unencumbered NYS license as a Registered Professional Physical or Occupational Therapist, Bachelor's degree or higher.
Experience with the MDS, RUG's, Medicare guidelines and JCAHO criteria.
Geriatric experience preferred.
A current unrestricted driver's license and dependable transportation.
